Audio clips and Poster art are in English, but all of the anomalies are clearable without English.
The game does not go for a realistic look, but it is designed to be a scary environment. Some anomalies have small amounts of blood painted on the textures, you can be chased and killed by monsters, a handful of intentional jump scares, audio recordings of fake news stories discussing murder investigations, etc.
